I see #1 all the time on the 35th st. exit off/on 35W.
It should be noted that a lot of the bad "cross merge" activity that happens is occurring at interchanges that are grossly over-capacity for the way they were engineered. At the above example, even if everybody drove perfectly, you'd still have back-ups, because there's simply not enough space to effectively merge when there's any significant traffic present.
